在美国新奥尔良，优秀的警探丹尼?巴克斯特(约翰?塞纳饰)尽职的工作让他终究在罪犯的心目中成为不共戴天的仇人。为了给自己在逃避丹尼追捕中不幸被撞死的女友报仇，已然在国际上小有名气的恐怖分子迈尔斯?杰克逊(艾丹?吉伦饰)自然不会轻易咽下这口恶气，于是，他在出狱后便立刻对警探丹尼和他的女友茉莉?波特(阿什莉?斯科特饰)实施了报复行动。
　　而狡猾且变态的杰克逊则不仅只满足于绑架甚至杀害丹尼?巴克斯特的女人那样简单。他更是运用自己对法律和警察办案中等疏忽的漏洞，为丹尼和他的警察同伴们设置了一个包括12个难关的“解救游戏”。丹尼只有在最快的时间和线路内顺利通过这12个险恶甚至随时有可能丧命的难关，才有可能将自己心爱的女人从虎口中拯救出来。
　　于是，在经历了房子被炸毁、街头狂奔、电梯警魂以及从直升机上纵身一跃等等险象环生的考验之后，警探丹尼?巴克斯特终于看到了女友生还的希望
两年前的《海军陆战队员》让摔角王约翰?塞纳充分体验到了登上大银幕的刺激。虽说那部影片在导演约翰?鲍尼托的手中表现平平并未收到“一飞冲天”的效果，但摔角手约翰?塞纳的初次触电却已经引发了不少人的关注。其明星效益已然从一个成功的摔角手甚至歌手、时尚达人便成了一位可以在物欲横流的好莱坞发展的未来之星。显然，动作片专家雷尼?哈林已经看到了这一点。于是，在名门之下，约翰?塞纳在《12回合》中的演出则显现出更加的专业和老道。从而，人们也很难看到塞纳在《海军陆战队员》中略现做作的POSE，僵硬的表情，以及看上去笨拙可笑的粗胳膊粗厚背。而在雷尼的调教下，《12回合》中的摔角王已然可以表现出一位专业演员的迹象。大量的对话和自我表现，仿佛让人们看到了《虎胆龙威》系列中威利斯的影子。这无论对于塞纳本身和动作类型片来说都是件天大的好事。
　　硬汉、英雄自然少不了美女的陪衬。影片中出演警探丹尼女友的金发女是曾经在《威震八方》中与另一位摔角冠军巨石强森激情合作过的阿什莉?斯科特。此女乍看上去虽没有科尔丝腾?邓斯特那般可爱，莎莉兹?塞隆那般迷人，亦没有斯嘉莉?约翰逊那样性感。但斯科特身上的那种独特的气质则给人别样的感受。尤其是她在《碧海追踪》中大秀比基尼的演出之后，更让她成为很多好莱坞导演眼中喜爱的那种慢热型“花瓶”。不会在瞬间就夺取所有观众的眼球，且始终能成为人们关注的焦点之一。为约翰?塞纳这样一位大银幕陌生者配戏，阿什莉?斯科特再合适不过。
